How to register as a patient

We can register anyone living in the following Leeds postcodes:
LS 1, 2, 3, 4, 5, 6, 13 and 16

If you live just outside a postcode boundary it may still be possible to register, please discuss with the practice manager. It may be possible to remain registered if you are an existing patient who moves to a new address within a reasonable distance of the practice; please ask.

Students can register at the practice through term time and in addition can still see their home GP as a temporary patient during holidays.

Registering at the practice couldn't be easier:

  • either print off the GMS1 registration form, complete and bring into surgery
  • or call in and ask for a form at reception.

You will need your NHS number and previous GP details to complete the form.

We will also ask you to complete a practice questionnaire on your present health state and any family medical history. The reception staff will answer any questions you have in completing the forms.

How to book an appointment

Routine appointments

You can book an appointment by phone or in person. Appointments are 10 minutes long, however if you feel you need longer, please tell the receptionist when you book.

We offer the option to book appointments in advance with a doctor of your choice. We are trying to extend the range of appointment times to early mornings and later evenings to help those who work. Please be aware as some of our doctors work part-time and so can only offer appointments on certain days.

Urgent appointments

If you feel your problem cannot wait and you need urgent medical attention today then we provide same-day appointments. Just tell the receptionist when you request the appointment.

Demand for these appointments can be very high especially in the winter and on Monday mornings. Please use this service responsibly.

Remember local pharmacists are trained to give advice and treat many minor conditions such as coughs, colds and sore throats and can provide services such as emergency contraception and pregnancy testing.

Cancelling appointments

Up to 15% of patients who book an appointment then fail to turn up. This wastes doctors' time and leads to longer waiting times for other patients. Please tell us if you cannot make an appointment so the time can be used for someone else.

How to request a repeat prescription

Regular medications that have been agreed by your doctor can be requested in several ways without seeing a GP by using the white printed list given to you with each prescription:

For safety reasons we cannot accept telephone requests.

In each case please give us 24 hours (working day) notice to process your request.

Annual Medication Reviews

All patients on repeat prescriptions are requested to have a medication review by their GP at least once a year to ensure your safety and the best use of available treatments. If this review is overdue we reserve the right to delay further prescriptions until you have seen a GP.

How to request telephone advice

If you would like to medical advice or have been asked to phone for follow up by a GP, please call reception and leave a contact number for the doctor to return your call when available.

If your query can be solved by one of our support staff - such as chasing a hospital appointment - then please ask to speak to one of the team.

How to request a home visit

Home visits are strictly for people too ill to get to the surgery. If you think you need a home visit please try and call us before 10am to help the doctors plan their day's work.

How to obtain results from blood tests

If you have an abnormal test result we will endeavour to contact you, so please make sure we hold up-to-date contact details for you.

You can also obtain results by phoning our practice nurse between 12.30-1pm.

Recognising the role of carers

We recognise the vital and often invisible role of carers. Our practice has approximately 800 patients who also act as carers for partners, family members or friends. You may find it helpful to let us know you are a carer, or that you have a carer, so that we can take that into account when trying to help you. For example, you may need a doctor's appointment at a certain time to fit in with your caring responsibilities, and if we are aware of that we can do our best to help.

We can also refer you to our colleagues in Carers Leeds, a specialist voluntary organisation which provides advice, support and assistance to anyone who is a carer.

Ask any of our staff for more details or complete a "yellow card" (available at reception) to register as a carer at our practice.

Involving our patients

We believe in involving our patients in helping us plan the development of our practice. We are looking for patients and carers to join our Patient Participation Group. This meets four times a year with the Practice Manager and the GP Partners so we can hear your views on how we can continue to improve the services we provide. We cannot always promise to do everything we are asked but we will listen and will always try our best to take account of patients' views when we make decisions about our practice development.
